An extensive New Patient Form was developed to streamline business processes and provide better customer service. The New Patient Form required SSL encryption, client-side validation Javascripts, server side validation scripts and a dynamically generated CSS layout email. When a user enters their New Patient information an html email is sent to the office that is formatted for printing. This enables the office staff to have a completed New Patient Form that is ready to be signed by the patient during their initial visit.
